About Us

Within the PCMH we work in teams that include a doctor, psychologist, psychiatrist, nurse, nutritionist, medical case manager/care coordinator and you (the most important member of the team). When you join the medical home, you will be provided with information and assistance in choosing the physician that fits your needs. Your physician will lead the team in planning and taking care of you. They will all have access to your electronic medical record and will work in partnership with you and your family to give you the best care possible when you need it. We know that it can be difficult to understand the many moving parts of your care. Therefore, we provide each patient with a medical case manager/care coordinator that will help you navigate the health care system.

The AIDS Care Center (ACC) is one of the few NYSDOH Designated AIDS Centers (DAC) in the Hudson Valley region. We are a Family Practice that provides comprehensive outpatient primary care and supportive services for patients of all ages. Taking care of you as a whole person is what the AIDS Care Center (ACC) PCMH is about. This means that your doctor, who specializes in taking care of individuals with or at risk of contracting infections including HIV and Hepatitis, attends to your personal, medical and life circumstances rather than focusing on one condition, organ or system.

Some of the clinical services that we provide are:

  • HIV testing and treatment
  • Hepatitis C testing and treatment
  • PrEP (Pre-Exposure Prophylaxis) for individuals at high risk of contracting HIV
  • Occupational and Non-Occupational PEP (Post-Exposure Prophylaxis) for individuals who may have had possible HIV exposure within 72 hours
  • Infectious Disease Specialty Consultations
  • STD testing and treatment

The PCMH is part of the Westchester Medical Center which means you are connected to this region’s most advanced medical care and referral hospital. We are also a part of RHIO (Regional Health Information Organization) which means your health information is securely shared across practices with your written consent, assuring timeliness and accessibility of your care.

What is a Patient Centered Medical Home (PCMH)?
It is a way of providing primary health care in which a team of healthcare professionals get to know you, your family situation, your medical history and current health issues in order to work with you so that you get the best care possible. They provide you and your family access to medical advice 24/7. All services are provided to every individual regardless of ability to pay. If you are not able to pay for your medical care, you will be given information on how to get medical insurance.

What does “whole person” mean?
It means that your personal doctor is in charge of making sure all your care needs are addressed. If you need care that your personal doctor cannot provide, a member of the Medical Home team will help to ensure that you receive that care. This includes care for all stages of life; acute care; chronic care; behavioral health; preventive services and end of life care.

What is your Medical Home’s responsibility?

  • Help you address any health issues including behavioral health needs you have throughout your lifespan by providing complete care and self-management support.
  • Help you find resources in your community that benefit you and your family.
  • Offer services you want or need from any member of the medical team in a timely manner. The types of services you can expect to receive include behavioral health services, specialty referrals, and follow-up as well as care coordination.
  • Offer assistance in addressing things that may negatively affect your health.
  • Help you in building a care plan that addresses your health and lifestyle goals.

What will your Medical Home team do?

  • Get to know you, your family situation, your life situation and likes/dislikes.
  • Treat you as a full partner in your care.
  • Communicate with you in a way that ensures we both understand each other.
  • Give you time to ask questions and answer them in a way that you can easily understand.
  • Make sure that you understand all your options for care.
  • Help you decide what care is right for you.
  • Ask you what you think about your experience.
  • Provide you with evidence-based care and self-management support.
  • As a receiving provider, identify the responsible party for medical decision-making.
  • Explain consent and confidentiality policies to you, your family and other caregivers.
  • Maintain ongoing collaboration between you, your family and the interdisciplinary team which facilitates the process of informed decision-making.

What does evidence-based care mean?
Evidence-based care generally refers to approaches to prevention and/or treatment that are validated by some form of documented scientific evidence. In other words, it means the methods or practice has been tested and found to work well.

What does self-management support mean?
In addition to helping you manage your condition, your Medical Home team will provide you with self-management support. Self-management means the tools, training, and skills by which patients with a chronic condition, disability, or disease can effectively take care of themselves. Your PCMH team will provide you with the support in the form of education, encouragement and helping you navigate through the various resources you need in order to take care of yourself.

Who is your Medical Home team?
Your team includes a doctor, nurse/LPN, nutritionist, care coordinator, social worker and a therapist. They will help you get healthy, stay healthy and get the care and services that are right for you.

What is your role as the patient?

  • Provide us with a history of your health and the health of your family.
  • Tell us about all the prescription and non-prescription medications you are taking.
  • Make sure that you tell us if you have recently seen any other doctors, gone to the ER or were admitted to any hospital or other facility such as rehab or nursing home.
  • Provide a point of contact to help coordinate any transition of care.
  • Bring all your medications (including medications that were not prescribed by your doctor) with you to every visit. If your medication needs to stay in your refrigerator, do not bring them. You can just write on a piece of paper the name, how you take them and when.
  • Learn about your health problems and take an active role in your care.
  • Try to follow the plan that you and your medical team have agreed upon.
  • If you are struggling with the care plan, talk to us about it.

How do I access medical advice during and after office hours?
During office hours: Please call (914) 493-7700 and someone will answer. If no one picks up, it is because they are on another call or with a patient. Please leave your name, number, date of birth and reason for your call on the voicemail and someone will call you back with clinical advice within one hour. If your call is about medication renewals and is placed before 4pm, you will receive a call back before the end of the business day.

After office hours, weekends and on holidays: Please call our answering service line at (914) 493-7649 and someone will answer. They will ask for your name, number, date of birth and reason for your call and give the information to a doctor who will then call you within two hours of your call.
